STRESZCZENIE
Neuroborelioza (NB) to jedna z postaci choroby z Lyme, manifestująca się różnorodnymi objawami ze strony układu nerwowego. Mogą one pojawić się w okresie od kilku dni do wielu lat od zakażenia. W Oddziale Neurologii Dziecięcej w Chorzowie w latach 2001–2010 NB rozpoznano u 21 dzieci w wieku 3–17,5 lat (śr. 10,6 lat). W stadium wczesnym rozsianym chorobę stwierdzono u 17 (82%) pacjentów: w 11 (52%) przypadkach pod postacią izolowanego obwodowego porażenia n. VII, w 2 (9%) przypadkach było to limfocytarne zapalenie opon m-rdz. (w jednym także z porażeniem obwodowym n. VII), u jednego (4,5%) dziecka zapalenie opon i zapalenie wielokorzeniowo- nerwowe, u dwojga (9%) dzieci rozpoznano zespół Bannwartha, a u jednego (4,5%) pacjenta zapalenie mózgu z poliradikulopatią aksonalną ruchową i porażeniem wiotkim czterokończynowym. U dwojga dzieci (9%) dzieci rozpoznano NB w stadium późnym pod postacią niedowładu połowiczego (u jednego z towarzyszącym porażeniem obwodowym n. VII) oraz ze stwierdzeniem zmian demielinizacyjnych mózgu w badaniu MR. U dwojga (9%) dzieci postać choroby (wczesna/późna) była trudna do określenia – u jednego stwierdzono obustronne obwodowe porażenie n. VII z towarzyszącą poliradikulopatią demielinizacyjną, u kolejnego izolowane obwodowe porażenie n. VII ze zmianami demielinizacyjnymi w mózgu w obrazie MR. U wszystkich dzieci wykazano obecność swoistych przeciwciał p. Borrelia burgdorferi (pBb) (ELISA) we krwi i/lub w pmrdz. U trojga wykonano także badanie metodą Western blot. Zwraca uwagę fakt, że wszystkie postacie wczesne rozpoznawane były w okresie od czerwca do października. U czworga dzieci z obwodowym porażeniem n. VII przed hospitalizacją ambulatoryjnie stosowano w leczeniu sterydy z zasady przeciwwskazane w NB (opóźnianie serokonwersji i przedłużanie występowania objawów chorobowych). W tym samym okresie w Oddziale Neurologii hospitalizowano 59 dzieci z obwodowym porażeniem n. VII, u których wykluczono NB.

ABSTRACT
Neuroborreliosis is one of the manifestations of Lyme disease, presenting a wide range of neurological symptoms. They may occur within several days up to many years after the infection. In the Child Neurology Ward in Chorzów in the years 2001– 2010, 21 children aged 3–17,5 (mean 10,6years) were diagnosed with neuroborreliosis. In the early disseminated stage the disease was diagnosed in 17 patients (82%): in 11 cases (52%) as isolated peripheral facial paresis, in 2 children (9%) as lymphocytic meningitis (in 1 child together with peripheral facial nerve palsy), 1 child (4,5%) with meningitis and acute radiculoneuritis, 2 patients (9%) were diagnosed with Bannwarth syndrome and 1 (4,5%) with meningitis with axonal polyradiculopathy and tetraplegia. 2 children (9%) were diagnosed with neuroborreliosis in the later stage, the symptoms were hemiparesis with demyelination in MRI scans (in 1 patient accompanied by peripheral facial nerve paresis). In 2 (9%) patients it was difficult to determine whether the disease was at the early or late stage. In 1 bilateral peripheral facial nerve paresis was observed accompanied by demyelination MRI scans. Antibodies against Borrelia burgdorferi (ELISA) in blood and/or CSF were found in all children. In 3 patients Western blotting tests were also performed. It is significant that all cases of the early stage of the disease were diagnosed between June and October. Before hospital admission, 4 children with peripheral facial paresis were treated with steroids, which are contraindicated in neuroborreliosis (they delay seroconversion and prolong the symptoms of the disease). In the same period of time time 59 children with facial nerve paresis were hospitalized the Child Neurology Ward; neuroborreliosis was excluded in those patients.
